Introduction to the Social Work Office

Individuals or families receiving medical care at our hospital and facing financial hardship may submit relevant supporting documents to our institution. Upon evaluation and review by the Social Work Office, those who meet the subsidy criteria will be provided with medical-related assistance, including but not limited to the following:

  • Organ Donation, Hospice and Palliative Care, Advance Care Planning Consultation, and Public Awareness Campaigns

Guidelines for Applying for Social Services and Emergency Relief Assistance Program (110年03月01日起)

  • (一)Recipients of Assistance:
  • Individuals or families receiving medical care at our hospital and facing financial hardship may submit relevant supporting documents to our institution. Upon evaluation and review by the Social Work Office, those who meet the subsidy criteria will be provided with medical-related assistance, including but not limited to the following:
    1. Individuals or households certified by local governments as low-income, middle-to-low-income, and elderly persons living alone who are receiving middle-to-low-income senior living allowances。
    2. Marginal households" refer to individuals or families who have applied for government welfare programs or insurance benefits and are currently under review, or those who do not meet the eligibility criteria but have actual and urgent needs. Those whose total medical treatment expenses exceed one-third of the total annual household income. Those whose average monthly household income per person is lower than 1.5 times the standard for personal living expenses set by the local municipal or county government, and whose family is facing an emergency situation.。
    3. Special Cases: Individuals assessed by professional social workers from affiliated hospitals as having verified medical care needs, or those referred by social workers from other social welfare organizations or government agencies。
  • (二)Subsidy Items:
    1. Inpatient and Outpatient Medical Expenses。
    2. Temporary Caregiver Expenses During Hospitalization。
    3. Others: Medical-related care expenses such as assistive devices, medical supplies, consumable materials, ambulance transportation, emergency assistance, and other related costs。
  • (三)Eligibility Criteria and Standards for Assistance:
    1. Applicants must first pay for in-hospital medical expenses or other caregiving costs. Original or copy of the official receipt must be provided as supporting documentation within two months from the hospitalization period or discharge date。
    2. Partial subsidies will be provided based on the approved subsidy amount。
    3. Eligibility for the subsidy must be determined through a preliminary assessment conducted by professional social workers of the hospital。
    4. During the review process, the subsidy will be granted only after final approval by the Chairperson of the Hospital Management Committee。
  • (四)Required Supporting Documents and Materials:
    1. An original copy of the updated Household Registration Transcript (listing all family members), issued within the past three months. (The transcript must include current residents as well as non-resident members under the same household head, with detailed annotations. Please apply at the Household Registration Office.)
    2. A photocopy of the official certificate issued by the Social Affairs Bureau or District Office of each city or county, verifying status as a low-income household, middle-to-low-income household, or recipient of middle-to-low-income senior allowance, or other relevant social welfare status. (Not required if unavailable.)
    3. An original copy of the household’s most recent annual consolidated income tax statement (including all categories of income) and the property ownership statement. (These documents must be applied for at the National Taxation Bureau or its branches or local tax offices. Not required for those holding official low-income, middle-to-low-income, or middle-to-low-income senior allowance certificates issued by the Social Affairs Bureau or District Office.)
    4. Copies (front and back) of other relevant supporting documents: Required if the applicant or any family member holds any special status. (For example: disability identification booklet, student ID, incarceration or release certificate, housing lease agreement, unemployment certification, low-income or financial hardship certificate, etc.)
    5. A photocopy of the applicant’s bankbook (passbook). (If a family member's bank account is used for receiving the subsidy, a photocopy (front and back) of the family member’s National ID card and a letter of authorization for receiving the payment must be submitted.)
    6. A photocopy (front and back) of the applicant’s National Identification Card. For new residents, a photocopy (front and back) of either the National Identification Card, Alien Permanent Resident Certificate, or Long-Term/ Permanent Residence Permit for new immigrants or foreign migrant workers is required。
  • (五)Assistance Application Process:
    1. The patient or family member must submit the application during hospitalization or within two months from the date of discharge. (Please refer to Item 4 — Required Documents. Incomplete documentation will result in disqualification from applying.)
    2. Social worker's home visit/interview and completion of the subsidy application form。
    3. Review of application documents. (The review process will take approximately one month.)。
    4. The result of the review will be communicated to the applicant or their authorized representative。
    5. For approved applicants, the subsidy amount will be transferred to the bank account (or other account) provided during the interview within three months from the date of approval, based on the approved subsidy amount。
